Passport and Passport Photo Services at NHFPL

Renewing your passport is quick, easy, and convenient at the New Haven Free Public Library.

We provide services for the application of US passports, passport cards, and both US and international passport photos.

The Passport Office will be closed on Saturday, June 8.


Open Hours: Saturdays 11-3:30 pm

Final appointments for each day are made 30 minutes prior to the closing of the Passport Office. Appointments made in advance are recommended. Walk-ins are taken as availability allows.


All forms should be completed ahead of time when possible. Forms are available at the Ives Main Library or can be printed by Library staff free of charge. All applicants must appear in person, including children.

Please note that adults with a ten-year passport must renew by mail using form DS-82; assistance can be provided. If you were 15 or younger when your most recent passport was issued, you must come in to renew your passport in person. Renewal forms may be downloaded at travel.state.gov


What You Will Need

ADULTS (AGES 16 & OVER)

  • One passport photo (photo services available for $15 or $18 to also receive a digital copy).
  • Original birth certificate (with raised seal) and social security number, OR naturalization certificate, OR old passport.
  • Valid driver’s license (copy of front and back) OR a US or foreign passport.
  • Out-of-state driver’s license must submit a second form of ID
  • Personal or bank checks (no cash accepted for US Department of State).
  • Proof of name change (marriage certificate, court order for name change, or divorce decree).

AGES 16 – 18

  • Bring your own ID OR bring one parent/guardian to identify you.
  • If alone, you may be asked to provide signed, notarized consent of one legal guardian. Call for more information.

CHILDREN 15 AND YOUNGER

  • Original birth certificate (with raised seal) and social security number OR old passport and photocopy of birth certificate OR naturalization certificate.
  • Both parents/legal guardians must be present or submit a notarized, written consent statement by non-appearing parent with a copy of his or her driver’s license (front and back).

You must pay separately for the application and photo fees.

Payment to the New Haven Free Public Library can be made by check, money order, cash, credit card, or debit card. Please make checks payable to: New Haven Free Public Library. Payment to the US Department of State must be by check or money order.
